Appointments
You may schedule an appointment by calling, (615) 515-1900 (local), 1-(866) 515-0019 (toll free), or request an appointment online.
One of our Patient Service Representatives will be happy to assist you with scheduling an appointment to see one of our physicians.
New Patients
For your first visit, please bring your insurance card(s), a photo Id, and plan to arrive early to complete the necessary patient information forms. You may also download our new patient registration forms in advance and bring the completed forms with you when you come for your appointment.
What to Bring With You for Your Visit
- Medications in their original containers
- Any test results that are obtained from the referring M.D.-examples:
- EKG
- Holter monitor
- stress test results
- letters
- and past medical histories.
- Any allergies to medications
- Insurance cards
- Referral for treatment from the primary care M.D.
What to do in an Emergency
If You Experience Chest Pain:
If you have nitroglycerin to use it would be appropriate to put one under your tongue. If in 5 minutes the pain persists, then repeat the nitroglycerin. At this point, consider proceeding to the nearest emergency room. If you do not have nitroglycerin to use, please proceed to the nearest emergency room.
It is important to emphasize that you go to the nearest emergency room and not attempt a long a long drive to Centennial Heart. If you are having a heart attack, any emergency room facility can make that determination and contact our physicians for transfer arrangements. It is important to realize that you need to be stabilized as soon as possible and that time is of the essence.
